Phase 2 - Full Proposal

Organizations whose Letters of Intent have been approved may submit a Full Proposal. The request must be specific and include:

  1. The Full Proposal Form
  2. completed "Vision Worksheet"

Additional materials required:

  1. Copy of last audited financial statement
  2. Organization's Annual Budget
  3. Copy of most recent 990
  4. Copy of 501(c)3 letter or letter of fiscal sponsorship
  5. Recent press and/or publications
  6. Resumes of key staff members
  7. List of Board Members and their affiliation
  8. A complete list of secured and potential funding sources (grants received, requests pending, planned submissions, etc.)

We may request additional information from applicants and consult with persons knowledgeable about the organization and/or the proposed activities. As part of the review process, we prefer to meet in person. Approved proposals will be sent to the Board on a rolling basis for a final determination. Final decisions on grants are made within two months of receipt of a proposal.
